Advanced Nurse Practitioner
Legacy Care Medical is a multiple provider practice with four office locations in the Northwest Suburbs of Chicago. Legacy Care Medical (FQHC Look- A-Like) is a remarkable medical practice committed to offering the communities it serves high quality, affordable and respectful care. We provide primary care for pediatric, OB/GYNE and family practice patients. The Advanced Nurse Practitioner position will be working in our Hanover Park clinic.

Responsibilities
-
Take, evaluate, and record medical histories.
-
Perform physical examinations required to evaluate medical problems.
-
Perform health maintenance for well patients, including administering or supervising immunizations.
-
Treat problems within the scope of competence and exercise judgment on problems requiring consultation, referral, or evaluation.

Manage illnesses
-
Collect specimens for pathological examinations
-
Prescribe medication as determined by the formulary and best practice
-
Counsel patients on medical problems, use of medications, expected effects of treatments, diet, and other health maintenance matters.

Counsel family planning, including prescribing of oral contraceptives
-
Manage behavioral or psychosocial problems including crisis intervention and short-term individual and family counseling
-
Order and interpret appropriate laboratory studies, x-rays, electrocardiograms, and other special examinations
-
Analyze and interpret data, formulate diagnoses and problem lists, and establish plans for the management of health problems
-
Initiate consultation requests to specialists and other health professionals including physical therapists, occupational therapists and dietitians
-
Participate, record, respond and acknowledge the On-Call phone system
-
Participate and perform all skills and duties for the hospital rounding schedule (refer to Employment Agreement)

Maintain all CME (continuing medical education) required

Have a method of transportation to travel when requested
Knowledge
-
Knowledge of administrative and clerical procedures and systems such as word processing systems, filing and records management systems, forms, and other office procedures

Knowledge of computer hardware and software, including applications and clinical programming
-
Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar
-
Knowledge of principles and processes for providing customer and personal services including needs assessment techniques, quality service standards, and customer satisfaction evaluation techniques

Knowledge of the CDC immunization schedule, preparation and timely disposal, and safe use of vaccines, needles and syringes
