We are seeking a compassionate, experienced Family Nurse Practitioner (FNP) who shares our commitment to clinical excellence, health equity, and outstanding patient care. Position Summary

The Family Nurse Practitioner provides comprehensive primary care services to pediatric, adolescent, adult, and geriatric patients. Responsibilities include health promotion, disease prevention, diagnosis, treatment, and management of acute and chronic illnesses while practicing within the scope of licensure and organizational policies.

Essential Duties and Responsibilities

  • Obtain and document comprehensive medical histories.

  • Perform complete physical examinations and health assessments.

  • Diagnose and treat acute and chronic medical conditions.

  • Provide preventive care, wellness visits, and age-appropriate screenings.

  • Administer/supervise vaccine administration according to CDC guidelines.

  • Order, interpret, and evaluate laboratory tests, diagnostic studies, EKGs, and imaging.

  • Develop individualized treatment plans based on evidence-based practice.

  • Prescribe medications and therapies in accordance with state regulations and organizational protocols.

  • Counsel patients regarding disease prevention, nutrition, medications, healthy lifestyles, and treatment plans.

  • Assess and manage behavioral health concerns within scope of practice and coordinate referrals as appropriate.

  • Coordinate referrals and collaborate with specialists and ancillary healthcare providers.

  • Maintain accurate, timely, and complete electronic medical records.

  • Participate in quality improvement initiatives and organizational performance goals.

  • Deliver culturally competent, patient-centered care while maintaining patient confidentiality and HIPAA compliance.

Additional Responsibilities

  • Participate in the organization's rotating on-call schedule.

  • Maintain required Continuing Medical Education (CME) credits and professional licensure.

  • Attend provider meetings, staff meetings, and required organizational training.

  • Support quality improvement, compliance, and patient satisfaction initiatives.

  • Adhere to HRSA, FQHC, OSHA, HIPAA, and organizational policies and procedures.

Qualifications

Required

  • Current, unrestricted Illinois Advanced Practice Registered Nurse (APRN) License.

  • Current Illinois Registered Nurse (RN) License.

  • Board Certification as a Family Nurse Practitioner.

  • Current DEA Registration and Illinois Controlled Substance License (or ability to obtain).

  • Full Practice Authority (or ability to obtain).

  • Current Basic Life Support (BLS) Certification.

  • Eligible for credentialing with Medicare, Medicaid, and commercial insurance plans.

  • Proficient in Electronic Health Records (EHR).

Preferred

  • Two (2) or more years of Family Nurse Practitioner experience in primary care.

  • Experience working in an FQHC, community health center, or underserved population.

  • Bilingual (English/Spanish) strongly preferred.

​

Knowledge, Skills, and Abilities

  • Strong clinical assessment, diagnostic, and treatment skills.

  • Knowledge of evidence-based primary care guidelines.

  • Familiarity with CDC immunization schedules and preventive health recommendations.

  • Excellent interpersonal and patient education skills.

  • Ability to work independently while collaborating effectively within a multidisciplinary care team.

  • Strong organizational, documentation, and time management skills.

  • Excellent verbal and written communication skills.

  • Ability to prioritize multiple responsibilities in a fast-paced clinical environment.

  • Commitment to cultural sensitivity, health equity, and exceptional customer service.

  • Proficiency with Microsoft Office and Electronic Health Record systems.

Job Type: Full-time

Pay: $112,000.00 - $135,000.00 per year
