Behavioral Health Program Assistant
Full-time Program Assistant position available for the daily management and operations of behavioral health services of Legacy Medical Care (LMC). The Program Assistant coordinates the program and patients’ needs as well as receptionist/administrative duties. Ensures full compliance with the LMC policies and procedures and Federally Qualified Health Center (FQHC) guidelines.

TASK AND DUTIES:

Handle sensitive and personal information with an understanding and respect for consumer confidentiality

Provide documented consumer engagement, follow-up, and data-entry to capture consumer referrals and outcomes
-
Achieve performance targets, working closely with the Navigator team, program Assistant and consortium partners to identify and overcome challenges
-
Remind potential participants of appointments through reminder postcards and calls and track all consumer contacts
-
Assists in ensuring clinic meets established productivity standards and works with clinicians to determine what prevents the clinician from making productivity, and the development and monitoring of a plan
-
Serve as the liaison between the patient and the behavioral health specialist

Document enrollment and educational
-
Track enrollment and education efforts and provide accurate program reports on time

Work with a team to rotate some evening and some weekends when needed
-
Other duties as assigned
RESPONSIBILITIES:
-
Responsible for administrative matters, such as scheduling and arranging appointments, answering, and responding to patient calls and or messages

Responsible for greeting and assisting patients and the parents of patients completing the registration process

Responsible for participating in ongoing training
-
Responsible identifying service gaps and advocates and coordinates with primary care providers
-
Responsible represents the behavioral health department in meetings

KNOWLEDGE, ABILITIES, AND SKILLS:

Strong communication skills (oral and written) and ability to interact professionally with a variety of stakeholders including patients, families, health professionals, community agencies, and government departments.

Demonstrated ability to work with people of diverse racial, ethnic, and economic backgrounds.

Knowledge of the health and human services infrastructure, health insurance programs and public coverage options.
-
Strong computer skills and ability to quickly learn new software programs.

Detail oriented and organized.
-
Ability to work independently and as part of a group to achieve program outcomes.
-
Leadership experience working with patients, coordinating events, and leading and facilitating groups.

Interest in community health/health equity.

EDUCATION AND/OR EXPERIENCE:
-
High school diploma/GED with 2-3 years of working experience required. Some college education with -2 years of working experience preferred.

minimal Two plus years of customer service experience
-
Ability to travel with use of a personal vehicle with current and valid Illinois state driver’s license and auto insurance.

Willing and able to work some evenings and weekends as needed, with advanced notice
-
Preferable experience working with behavioral health/medical office
